A completed medical history form is necessary to obtain the basic information needed to provide you with the care that is best suited to your needs.

Therefore, take care in completing it with as much detail as possible; it will help your doctor better know your personal medical history, individual needs, medications, and allergies. It is SUPER important!

Please be aware that these may not be the only forms you will need to fill out if you have an appointment with one of our providers and you may need to come in early to complete them prior to your appointment time. You will also need to bring any insurance information, be it medical, workman's compensation or from a motor vehicle accident.

 

Also be prepared to supply a list of medications and the current dosages that you are currently taking. If you fail to bring in any of this information or are too late for your appointment we do reserve the right to reschedule your appointment to another time.
